The Office of the Registrar is responsible for maintaining all academic records of Concordia University Chicago. We strive to provide prompt, courteous, and accurate service. This office is happy to assist you with transcripts, enrollment verifications, change of address, change of name, graduate registration, degree completion, and other record questions. The Office of the Registrar is located on the main floor of Addison Hall in room 155.
Regular business hours are as follows with exceptions during different times of the semester and over holiday breaks: Monday and Friday: 8 a.m. – 4:30 p.m. and Tuesday, Wednesday and Thursday: 8 a.m. – 6 p.m.

Forms for current students, faculty, and advisors such as the stop-out and withdrawal form can be found at Concordia Connect. Once logged on, go to the “Resources” tab, and search for "Registrar" to find the necessary documents. Most forms are also available in the Registrar’s Office in hard copy format. Forms are located at the bottom of this page for those who do not have access to Concordia Connect are known as "guest students" which can include but are not limited to alumni and high school students.
